Read the following original passage and write a précis of approximately one-third of its length. Supply a suitable title.
Institutions often hide failure because officials fear blame, yet concealed errors are likely to recur. Learning requires reliable records, independent evaluation, and a culture that distinguishes good-faith experimentation from negligence or misconduct. Accountability remains necessary, but punishment alone can encourage defensive behaviour and distorted reporting. After a policy failure, investigators should ask what assumptions proved false, which warnings were ignored, how incentives shaped conduct, and what safeguards can prevent repetition. The objective is not merely to identify a person to blame but to improve the system while preserving responsibility.
